• participate in general ward rounds; maintain accessibility with medical professionals regarding the psycho-social needs of patients
• provide casework management for patients who may be experiencing personal, emotional and environmental concerns
• assist patients and their families to adjust to chronic illness or disability and encourage them to play an active role in their medical treatment
• facilitate family conferences and information sharing and contribute to ongoing management of care plans
• work as part of a multidisciplinary team in interpreting the special needs of patients and make recommendations towards discharge plans
• could be placed in mental health, drug and alcohol, aged care, emergency, maternity wards
• attend relevant professional development training to maintain currency of practice.
NSW Health employs both graduate and non-graduate Welfare Officers. Eligibility varies accordingly.
Certificate IV, Diploma or Bachelor level as deemed appropriate by the employer. Membership of the Australian Community Workers Association may also be an employer requirement - at minimum.
